Consent Form
Before booking and attending a session please read through Kick Off Football Academy’s consent form. Please bring a signed copy to the player’s first session or alternatively email a signed copy of the form to kickoffacademymk@gmail.com. Without completing and signing the consent form the player will not be able to participate in the booked session.
PLEASE NOTE: FOR OUR INDIVIDUAL SESSIONS, YOU HAVE 48 HOURS PRIOR TO YOUR SESSION TO CANCEL. IF YOU CANCEL AFTER THAT TIME, A £10 FEE WILL STILL APPLY TO YOUR BOOKING
